Some notes for parents:

 - We do not keep any medication at school for student use. Any medication administered to your child must be provided by you or your doctor with written instructions and consent. The necessary form(s) can be downloaded below.   

 - All medication (including cough drops) must be kept in the original packaging with all instructions clearly visible including the expiration date. Make sure the medication is age appropriate. Read the label--if it exceeds the recommended dosage for your child's age/weight, or is an adult dosage, it will not be given, unless it is accompanied by a doctor's order (filled out on the Prescription Medication Form). DO NOT SEND IN LOOSE PILLS! All expired medication will be sent home. It is the parent's responsibility to replace the medication.
